Care Coordination Vision Statement
Our vision is to develop and foster a system of care that goes beyond the
concept of clinical integration of services. This system of care
provides a platform for health and wellness that connects essential elements
of peoples’ lives: clinical care, health promotion, recreation, social
service, education, employment, housing, and civic involvement. We
recognize the impact of social and economic circumstances on health and
wellness.
